Form 1095 Availability for Former Employees
In accordance with requirements under the Internal Revenue Service (IRS) and the Affordable Care Act (ACA), Form 1095 is available upon request for individuals who were eligible for our employer-sponsored health plan during the applicable tax year. Form 1095 provides information about health coverage that may be required when filing your federal income tax return.
