We offer:

Competitive wages based on annual market evaluations.

Retirement plans through Utah Retirement Systems for full-time and part-time employees.

Group benefits are effective the first day of the month following the hire date. Our benefits include 80% paid medical and dental premiums, a health savings account (with medical insurance), and 100% paid basic life, AD&D, and disability insurance.

We also offer voluntary vision, life, and accident insurance options.

Herriman City is a Social Security–exempt employer and offers deferred compensation (457 and 401k) plans with a tiered matching plan.

Form 1095 Availability for Former Employees

In accordance with requirements under the Internal Revenue Service (IRS) and the Affordable Care Act (ACA), Form 1095 is available upon request for individuals who were eligible for our employer-sponsored health plan during the applicable tax year. Form 1095 provides information about health coverage that may be required when filing your federal income tax return.

How to Request Your Form 1095

If you are a former employee and would like to receive a copy of your Form 1095, please submit your request using one of the methods below:
