Forms and Information
Police records are governed by GRAMA law. You may make your request in person, online, or by mail. Herriman Police has up to 10 business days to respond to your request and will contact you when your request is ready. For payment, please see the fee schedule. We will notify you of the payment method upon request completion.
Witness Statements
Witness statements should be printed out, completed, signed, dated, and then emailed to the officer or investigator in charge of the case.
